Please note: Upon receiving this application the Golf Office of Casa de Campo will contact the players for their credit card information to hold the reservations. Once provided they will authorize Casa de Campo to charge the entire package to their credit card starting Nov 21st, 2012 for the first two events and Dec 5th, 2012 for the final Caribbean International II. If any in your group fail to cancel on or before Nov 21st, 2012 for the first two events and Dec 5th,, 2012 for the final Caribbean International II, or do not show, a $250.00 US cancellation penalty per person will be deducted from the cost of the total package and the balance will be refunded. Teams must be complete to participate.
